As we age, our skin doesn't simply develop wrinkles. The underlying architecture of the skin changes as well. Collagen production declines, the extracellular matrix changes, and the skin gradually loses some of its firmness, thickness, and structural support.
This has led to growing interest in biostimulatory injectables—treatments designed not simply to add volume, but to encourage the body's own tissue-remodeling response.

Sculptra contains poly-L-lactic acid (PLLA).
Rather than functioning primarily as an immediate volumizing filler, PLLA is designed to produce a gradual tissue response. Human biopsy research has demonstrated increases in both type I and type III collagen following PLLA treatment, with statistically significant increases in type I collagen at three and six months in one human study.
This helps explain the gradual nature of Sculptra®'s results.
The treatment does not simply place collagen into the skin. Instead, the PLLA particles initiate a tissue response that involves fibroblasts and extracellular matrix remodeling.
In other words, Sculptra® provides a stimulus for the patient's own tissue to remodel over time.
RADIESSE® is based on calcium hydroxylapatite (CaHA).
CaHA has a somewhat different mechanism and can provide an immediate volumizing or structural effect from its gel carrier while also being associated with a longer-term tissue response.
Human histologic research has demonstrated new collagen formation following CaHA treatment. In one study, investigators identified an increase in newly formed type III collagen two months after treatment.
Another human split-face study provides particularly interesting insight into the remodeling process. At four months, type III collagen was greater following CaHA treatment compared with hyaluronic acid. By nine months, type I collagen staining was higher with CaHA, while type III collagen was lower. The researchers described this as a two-step remodeling process in which type I collagen gradually replaced type III collagen.
This is an important distinction when discussing biostimulators: the goal isn't simply to “make more collagen.” The tissue is undergoing a dynamic remodeling process.
The simplest answer is: both Sculptra® and RADIESSE® have evidence of stimulating type I and type III collagen, but the research and timing of the response differ.
With Sculptra® (PLLA), human tissue studies have demonstrated increases in type I and type III collagen, with particularly strong evidence for increased type I collagen at three and six months.
With RADIESSE® (CaHA), human studies demonstrate new collagen formation, including an increase in type III collagen during an earlier remodeling phase and greater type I collagen staining at a later time point.
This is one reason the term “collagen biostimulator” can be useful—but it is also important to remember that these treatments are not simply collagen injections. They interact with the tissue and encourage a biological remodeling response.
Think of the process less like filling a wrinkle and more like remodeling the foundation of the skin.
A traditional filler may be used to replace volume in a specific location. A biostimulatory treatment, depending on the product, formulation, treatment area, and technique, can also influence the tissue's own remodeling response.
The visible result therefore develops over time.
